Responsibilities:
RESPONSIBILITIES: The Registrar examines, numbers and signs birth and death certificates presented for filing to ensure that they conform to the filing requirements of the Public Health Law. A local copy of each document is maintained and the original is forwarded to the State Department of Health. The Registrar issues and files Burial Transit Permits and Disinterment Requests. Certifications, transcripts and certified copies of Birth and Death records are issued to qualified applicants. The Registrar keeps a count of the number of Fetal Death Reports received and destroyed. She presents supporting documentation with the Application for birth or death corrections to the Department of Health and amending the local copy. The Registrar issues uncertified copies from records of birth, death and marriage records provided for genealogical research purposes subject to the restrictions specified by the Health Commissioner’s rules and regulations. Complete records for genealogical research begin in 1887. The Registrar supplies blank forms ad certificates to physicians, funeral directors and nursing homes.
